Where sign-up goes wrong
A paper form scanned to a PDF, an email thread for questions, a separate payment link and a spreadsheet someone fills by hand. Each season the same family types the same address three times, and someone on the board spends a weekend matching payments to names. A good registration form asks each question once, checks the answers before they're sent and drops everything into one list.
What a registration form can handle
Season sign-up
Player details, age group by birth year, uniform size and the program they're joining.
Tryouts and camps
Pick a session or a camp week, with sessions closing when they reach the cap you set.
Waivers and policies
Club policies and waivers shown in full, with a checkbox and the parent's typed name.
Siblings
Add a second or third player without typing the family details again.
Payment links
A handoff to your payment processor or league platform once the form is complete.
Waitlists
When a group is full, the form offers the waitlist and tells the family where they stand.
How the form is built
We split long forms into short steps: the family, then each player, then the program, then policies, then payment. Questions change with the answers, so a family signing up for camp only sees camp questions. Age groups can be worked out from the birth date, which removes a common source of mistakes. The form saves progress on the device, so a parent who gets interrupted doesn't start over.
Children's details
Registration collects information about minors, so we keep the form to what the club needs. Parents fill it in, not players. Health notes are optional unless your league requires them, and they go only to the people who need to see them. We explain where the data is stored and who can reach it before you sign off, and we can set a date when old season records are cleared.
Where entries go
| Destination | Good for |
|---|---|
| A shared spreadsheet | Small clubs and camps run by a few volunteers |
| Email to the registrar | Clubs that review each entry by hand |
| Your league or registration platform | Clubs that must report players to a governing body |
| Your gym software | Gyms that sign up members or trial classes |
Questions
Can the form close a group when it's full?
Yes. Each group or camp week has a cap you set, and the form switches that choice to a waitlist once it's reached.
Can we reuse the form next season?
Yes. You copy the season, change the dates and groups, and open it again. Returning families can be asked to confirm details instead of typing them all.
